一、請提供115學年度第1學期學士班及碩士班各年級班級幹部聯繫資料，俾利學生安全輔導室處理學生事務緊急聯絡使用。

二、相關說明如次：

(一)單位：各學系、所及學位學程等學術單位。

(二)學制：學士班及碩士班(含在職專班)。

(三)職務名稱：班代及副班代。

(四)填寫期程：即日起至115年9月30日24時止。

1.Please provide the contact information of class officers for each year of the undergraduate and master's programs so that the Office of Student Safety Division can use it for emergency contact in handling student affairs.

2.The relevant explanations are as follows:

(1) Institution: Academic units including departments, institutes, and degree programs.

(2) Duration of Study: Bachelor's and Master's programs (including part-time programs).

(3) Job Titles: Class Representative and Deputy Class Representative.

(4) Application Period: From now until 24:00 on September 30, 2026.
