Dear New Students,

115學年度新生入學健康檢查預約名單已完成彙整，感謝同學耐心等候。請大家務必再次確認預約日期、時段及個人資料是否正確；如未完成預約、須更換時段，或另有校外健檢安排，請依下列說明辦理：
The appointment list for the 2026 New Student Health Examination has been finalized. Thank you for your patience. Please carefully check whether your appointment date, time slot, and personal information are correct. If you did not make an appointment, need to attend at a different time, or plan to complete the examination at an off-campus medical institution, please follow the instructions below:

一、新生健檢預約名單已公告，請同學再次確認預約日期、時段及個人資料是否正確。如資料有誤，請儘速來電或來信告知，以利協助處理。
1.The appointment list for the New Student Health Examination has been announced. Please check your appointment date, time slot, and personal information carefully. If you find any errors, please contact us by email or telephone as soon as possible so that we can assist you.

二、未預約到健檢時段，或因課表、選課及其他臨時要事無法依原時段受檢者，如仍希望 參加到校健檢，可於9月3日至9月5日、9月7日至9月8日，選擇可配合的日期及時段（每日最後報到時間為上午11時30分）直接前往惠蓀堂1樓學校報到處登記候補 。當日一定會協助同學完成健康檢查，但須依已預約學生的報到情形及現場空檔依序安排，等候時間確實無法事先預估，敬請預留充裕時間並多加見諒。
2.Students who were unable to reserve a time slot, or who cannot attend their original appointment due to class schedules, course registration, or other urgent matters, may still participate in the on-campus health examination. Please visit the school check-in counter on the first floor of Hui-Sun Hall on September 3–5 or September 7–8 at your preferred date and time to register as a walk-in.
All walk-in students will be assisted in completing the health examination on the same day. However, walk-in students will be accommodated according to the attendance of students with appointments and the availability of each examination station. Therefore, the waiting time cannot be estimated in advance. Please allow sufficient time and thank you for your understanding ,(The last check-in time each day is 11:30 AM).

三、原已完成預約，但因故需更換日期或時段者，無須重新上網預約，請改至現場候補區登記，並於候補資料上註明「原預約日期及時段」，以利工作人員查核名單及安排健檢。
3.If you have already made an appointment but need to change the date or time, you do not need to make another online reservation. Please register at the walk-in waiting area and clearly indicate your original appointment date and time on the walk-in registration form so that the staff can verify your information and arrange your examination.

四、若公司已安排健康檢查，或已於115年6月7日以後至其他醫療院所完成健檢，為避免短時間內重複受檢，可先依本校「學生健康資料卡」第2頁所列的檢查及檢驗項目逐項核對：
4.If your company has already scheduled a health examination for you, or if you completed a health examination at another medical institution on or after June 7, 2026, you may first compare the examination items with those listed on Page 2 of the University’s Student Health Information Card to avoid undergoing another examination within a short period.

• 項目及結果均符合本校規定者，可申請認列。
• If all required examination items and results meet the University’s requirements, the report may be submitted for review and recognition.
• 如有部分項目缺漏，可至合格醫療院所單項補檢，無須全部重新檢查。
• If any required items are missing, you may complete only the missing items at a qualified medical institution; you do not need to repeat the entire examination.
• 「口腔檢查」為較常見的缺漏項目，可至鄰近牙醫診所或本校特約醫療院所補做檢查。
• The oral examination is one of the items most frequently omitted. It may be completed at a nearby dental clinic or one of the University’s contracted medical institutions.

五、完成校外健檢或補檢後，請將下列資料各一份繳交至惠蓀堂1樓健康及諮商中心：
5.After completing an off-campus health examination or any additional examination items, please submit one original copy of each of the following documents to the Health and Counseling Center on the first floor of Hui-Sun Hall:

• 已完成健康檢查報告正本。
• The original health examination report.
• 缺項補檢報告正本（如有進行補檢）。
• The original report for any additional examination items, if applicable.
• 新生EZ Come系統填寫、列印並完成簽名的「學生健康資料卡」正本。
• The Student Health Information Card completed through the New Student EZ Come system, printed out and signed.

★建議於開學後一個月內（115年10月7日前）完成繳交，以利辦理健檢資料審核及登錄。★
Students are advised to submit all required documents within one month after the semester begins, preferably no later than October 7, 2026, so that the University can review and record the examination results.

六、原已預約到校健檢，但後續決定改至本校健檢承辦醫院「衛生福利部臺中醫院」或住家附近其他合格醫療院所辦理者，請務必來信或來電取消原預約，以利釋出名額並正確統計到校受檢人數，謝謝您的配合。
6.If you have already reserved an on-campus health examination but later decide to complete the examination at the University’s contracted hospital, Taichung Hospital, Ministry of Health and Welfare, or another qualified medical institution near your residence, please contact us by email or telephone to cancel your original appointment. This will allow us to release the appointment slot and accurately calculate the number of students attending the on-campus examination. Thank you for your cooperation.
